What personal information do we collect?
Information Your Doctor Provides Us. When your doctor registers you to use our services, you will receive a welcome email that directs you to this Privacy Policy. This information includes your name, email address, and date of birth.

Information You Provide To Us. If you communicate with our customer service team or send us an email, you are giving us information that we collect. That data may include your IP address, name, physical address, email address, phone number, and other details included within the correspondence. By providing us this information, you consent to collection, use, disclosure, and storage by us, only as described in our Terms of Use and Privacy Policy.

Information From Your Use Of The Service. We may get information about how and when you use the Services. This information may include your IP address, time, date, browser used, your computer's operating system, and, of course, the responses to a survey. This information is critical to ensure the services are working as intended.

Cookies. When you receive a survey, we store "cookies" which are small strings of code on your computer. We use those cookies only as a method to make the user experience more efficient such as a faster way to view your survey. Web Beacons. We use Mandrill as our email processor to send the survey links. This tracks when you open the email and click the link in order to determine when a survey is taken and to improve our features. To do this, Mandrill includes single pixel gifs, also called web beacons, in emails that are sent. Web beacons allow us to collect information about when you open the email, your IP address, your browser or email client type, and other similar details. We use the data from those Web Beacons to create the reports that helps us determine who has or has not opened emails or clicked on the survey link.

Email bounces around the internet from server to server. Along the way, system administrators could read what is sent. Email was not designed for confidential information. Because of the nature of our service, we strive to ensure we will do all we can to keep your information private - this includes surveys and responses to them. Since our services are web based, the actual content of surveys and the answers provided are not contained within the email (only the link). We require a login using a PIN the first time the service is used and with each different computer from which the website is accessed.

We do not sell, trade, or otherwise transfer to outside parties your personally identifiable information. We do not include or offer third party products or services on our website. We do not allow third party behavioral tracking.

The Fair Information Practices Principles provides the basis for confidence and trust for online transactions in the United States. In order to comply with Fair Information Practices we will take the following responsive action: Should a data breach ever occur, we will make every effort to notify users via email within 7 business days.

Safeguarding Your Information
No one can guarantee absolute privacy on the internet. We compartment our data to minimize any potential risk that may occur for reasons out of our control such as an unknown security flaw. Our credit card processing vendor uses the latest secure technology for secure transactions. These transactions are processed through a payment provider portal and are not stored or processed on our servers.

In order to access your personal information, you will need a username and password to log in. You must keep your username and password secure and never disclose it to a third party. These account passwords are encrypted, which means we cannot see your passwords. We cannot resend a forgotten password. We can only reset them.

CalOPPA is the first state law in the nation to require commercial websites and online services to post a privacy policy.
